    Rep. Van Roy: Dental Care Pilot Program

    Confirming what we have seen and heard anecdotally, the federal Centers for Medicaid and Medicare Services (CMS) recently ranked Wisconsin among the lowest states in the level of access to dental care for children on Medical Assistance (MA). According to many dentists, the rate at which state government reimburses them for the dental services they provide to MA patients is so low that many dentists are reluctant to accept any new MA patients and/or expand the number of MA patients they presently see. As a result, many Wisconsin residents who rely on MA programs have extremely limited access to dental services. Going without preventative care, these individuals develop problems and have to wait until the symptoms are so bad that they end up going to the hospital emergency room for help.

    Many of us in the legislature feel it is time for the state to test a theory that perhaps paying a market-based rate to dentists for the services they provide to MA patients will increase access to dental care. Over the winter, I worked with several other legislators to develop a bi-partisan proposal that would test this theory on a limited basis. Assembly Bill 927 is the result of those discussions. Our bill creates a pilot program in Brown, LaCrosse, and Racine Counties to increase MA reimbursement rates for all dental services provided to children on MA and for emergency dental services provided to adults on MA.

    In Brown and La Crosse counties, the services would be provided and reimbursed on a fee-for-service basis, and in Racine County the services would be provided and reimbursed through the medical HMO system. The project would remain in effect for two full years from the time the state receives approval for the pilot from the federal government. This should give us enough time to collect sufficient data to evaluate whether or not this approach is an appropriate public policy for improving access to dental care for this population of individuals. The pilot would automatically sunset after its two-year trial period. The Department of Health and Family Services would be required to file an interim report to the state legislature after the first 12 months of operation and a final report immediately after the program sunsets.

    Although Assembly Bill 927 was introduced too late in the session for it to make it through the entire legislative process, I believe it gets the discussion out there to help build support and momentum for passing the proposal when I re-introduce it in the 2009-2010 legislative session.
